At this time, we are only processing requests where required by applicable law.

Under certain circumstances, you may request that Fortis take specific action regarding your personal information, for example:

• Request the deletion or erasure of your personal information;
• Request a correction to your personal information;
• Request the transfer of your personal information; or
• Request to opt-out of the “sharing” (as defined in the CCPA) of your personal information.

Once you submit the form below, we will be in touch with you to verify your identity. If you are an agent submitting a request on behalf of another individual, we will also need to verify your authority to make the request. If we cannot verify your identity or authority, we will deny your request.
